i’m not really sure about the last 2.
The first word needed to be an adjective with two vowels:
vile, evil, veil, levi, live
I’m not entirely sure about the fourth word, but I think it might be as follows:

The only thing I could come up with in the third line was a name. Levi? Elvi?

OK, but I don’t think it very plausible that a sorceress would search the Old Testament for inspiration when choosing a name for her familiar.
